Do not use this form if your Blu-Ray menu loads in widescreen and you cannot get it to present video in 4:3 aspect; that is not a disk issue but an issue related to your TV which needs to have a Widescreen/Normal ratio selection button for you to be able to choose the picture format you want to use to view video.
Also do not use this form to request another copy of a Blu-Ray disk if the menu of a Blu-Ray loads as a data disk. This is not a disk issue either. If this happens, you need to view the Blu-Ray on a player with an internal data speed and/or memory more capable of processing larger files, or simply one that is capable of loading and playing BD-Rs.
We also do not provide replacements for DVD or Blu-Ray cases damaged in shipment. Go to Amazon.com or eBay and pick up some extra ones so you will have them on hand in case this happens, they are not expensive. If you are outside the US and need a disk, and intend to make another order soon, we appreciate it if you would notify us so your replacement disk might be shipped along with it. We will also ship replacement cover inserts with future orders if one has been damaged in shipment.
